11. Ongoing Follow-up

  • Continuous Monitoring: Use SCAT6/SCOAT6 tools to regularly reassess symptoms, cognitive function, balance, etc., and guide management progression. Adjust treatment plans based on patient progress and any new symptoms that emerge. Continuously realign the treatment plan with the patient’s evolving goals, feedback, outcomes, and clinical judgment.

  • Referral: Consider referral or co-management if symptoms persist beyond expected recovery timelines (2 weeks for adults, 4 weeks for children) or if new red or yellow flags arise. If the patient is making demonstrable improvements and no new concerns are present, ongoing management can be tailored on a case-by-case basis, with close monitoring of symptoms and functional progression.

  • Aligning with Patient Goals: Regularly revisit and adjust the patient’s initial goals to ensure they remain relevant and achievable. Address any barriers to goal achievement and adjust the treatment plan accordingly.
